What does discharge management mean for you as a patient?

Right at the beginning of your stay, you will be informed about our discharge management in the patient admission. In the further course of your stay, our physicians, nursing staff and case management will determine your individual care needs in order to assess restrictions on mobility and self-care and thus be able to make the necessary applications, prescriptions and appointments for rehabilitation, home care, nursing services, aids, short-term care or inpatient home care for you.

Legislation requires hospitals to ensure standardized discharge management from October 1, 2017.

It is also very important to us to offer our patients seamless further care - medical, rehabilitative and nursing - following their treatment in our hospital.

As part of our discharge management, we are particularly available to answer questions from the service providers who continue treatment.
