At the beginning of your stay, you will be informed about our discharge management in the patient admission department. During the course of your stay, our doctors, nursing staff and case management team will determine your individual care needs in order to assess any restrictions in your mobility and self-care so that they can make the necessary applications, prescriptions and appointments for rehabilitation, home care, nursing services, aids, short-term care or inpatient home care.
Legislation stipulates that hospitals must ensure standardized discharge management from 1 October 2017.
It is also very important to us to offer our patients seamless follow-up care - medical, rehabilitative and nursing - following their treatment in our clinic.
As part of our discharge management, we are particularly available to answer questions from the service providers providing further treatment.
